• 1-3 accidents or violations in the past three years
• OR, more then 3 accidents or violations in the past three years.

7) Proceeding to the eighth line is an enquiry about your blood pressure. Here you may again use the simple drop down menu to select the most suitable statement about the severity of your cardiovascular health, the following options have been provided:
• Normal blood pressure, checked regularly
• High blood pressure, controlled on medication
• OR, high blood pressure, not under control
8) Finally, you may choose to include some information about your family history of health on the ninth line, but it is not mandatory. Using the drop down menu some common selections have been provided so you may to best describe familial traits, such as:
• I don’t know
• Parents not yet 70 and in good health
• Both parents lived to age 70, no cardiovascular problems or diabetes before age of 60
• Cardiovascular problems, diabetes, or death before the age of 60
• OR, two or more siblings with cardiovascular problems or diabetes
